Please bring a valid Photo ID, Valid Medicare Card, DVA card, Health Care Card or Pension Concession Card and your insurance information (if applicable) to your first appointment. This helps expedite the registration process.
We kindly ask new patients to arrive at least 15 minutes before their scheduled appointment time to complete any necessary paperwork and registration.
