A mental health care plan, officially known as a Mental Health Treatment Plan, is a document your GP prepares if you have a diagnosed mental health condition requiring at least moderate care support. You get one by booking a standard appointment with your GP to discuss how you have been feeling, and if appropriate, they will prepare the plan and refer you on to a suitable mental health professional.

A mental health care plan is prepared under the Better Access initiative, a government program designed to help people access Medicare rebated mental health support. According to the Australian Government Department of Health, Disability and Ageing, the first step is always to speak with your GP, who will assess whether you are eligible for support under this initiative.
If your GP determines you have a clinically diagnosed mental health condition requiring at least moderate care, they will prepare your plan and refer you to a suitable mental health professional, such as a psychologist, social worker or occupational therapist.
Once your plan is in place, you may be eligible for Medicare rebates on mental health treatment sessions. According to Services Australia, eligible patients can claim a Medicare benefit for up to ten individual and ten group therapy sessions each calendar year, provided they have a valid plan and referral.
This does not mean every session is entirely free, since the rebate amount depends on the professional you see and their fees. However, it does significantly reduce the out of pocket cost compared to seeing a psychologist without a plan in place.
Getting a plan starts with booking an appointment with your GP and being open about what you are experiencing. Your doctor may ask some personal questions to understand your situation properly, which can feel confronting, but it helps them recommend the most appropriate support.
If you are registered with our practice through MyMedicare, your plan generally needs to be prepared and reviewed by a GP at our practice, since this supports continuity of care and ensures the same team stays across your progress over time.
Once your plan is ready, you can book in with the mental health professional your GP has referred you to. From there, it is a good idea to check in with your GP periodically, since plans are typically reviewed and can be adjusted if your needs change.
If you are also managing an ongoing physical health condition, this can sometimes be addressed at the same time through our chronic disease management service, particularly if your mental and physical health are connected.
